The Baltimore bridge collision minute by minute: Human error, failing power and bad luck

A long-awaited preliminary report charting the hours and minutes leading up to the fatal collision between the container vessel Dali and the Francis Scott Key Bridge in Baltimore in late March was released on Wednesday morning by the National Transportation Safety Board (NTSB).
